This form is only for requesting to freeze an EFT monthly membership. For those seeking an exception to the annual membership policy, please reach out to your gym and you will need to provide a doctors note in order to freeze an annual membership.

In order for us to process your freeze request for the upcoming month, this form must be submitted before 11:59pm PST on the 26th of the month.

EFT Memberships can be frozen for a minimum of 1 month, and a maximum of 3 months. Please choose a date that is at least 1 month after the start of your next billing cycle. By submitting this form, you acknowledge that you will be re-enrolled back into your EFT membership on the below date.
